✅Documents Required for Name change in Gazette of India:

Following are the list of documents required for name change in Gazette of India:

1. An undertaking duly signed by the applicant

✅Directorate of Printing Department of Publication Guidelines while Applying for Gazette of India: 

1. The matter of advertisement must be published in one of the daily local leading newspaper showing details of the case.
2. An undertaking duly signed by the applicant showing therein correct & wrong name along with full details of father’s/husband’s name with residential address declaring that the facts stated therein are true & correct to the best of his/her knowledge and belief.
In case of Indians living abroad, he/she will submit deed duly attested by the respective Indian Embassy/Indian High Commission, in original to this Department.
3. Printing matter as prescribed in the various attached specimen proforma duly complete in all respects by typing the same on a separate plain paper and signed by the individual in his / her name, with two witnesses in duplicate should be submitted along with Soft Copy (CD MS Word). The proforma must be computer typed.
4. The applicant is required to submit two self attested passport size photographs along with application with all the requisite documents containing two passport size photographs and copy of photo ID proof, both duly self attested.

The photocopy/cutting of a newspaper that is more than one year old is not acceptable. You may have to send the original newspaper after highlighting the advertisement (full page in which your advt. is published).

✅CD Format while making the Gazette of India:

A CD in MS Word (window 2007) format containing the matter of name change/Public notice without witness is required and should carry your old name in place of a signature on the CD. Please note that without a soft copy (CD), no action will be taken in your case.

Format for Change of name of Major-

• In place of a signature- Type your old name (no sign), and no witness details are required on the CD.
• Only standard specimen format for changing the name of the major is required. No other documents, such as application No, Photographs, or ID details are required in CD.

Format for Change of name of Minor-

• I, _____________S/o ______________, Residing at __________________, have changed the name of my minor son / daughter
  __________________, aged about_____ years and he/she shall hereafter be known as ___________.
  It is certified that I have complied with other legal requirements in this connection.
• In place of a signature- Type your Parent’s name (no sign), and no witness details are required on the CD.
• Only standard specimen format for changing the name of Minor is required. No other documents such as application No, photographs, Or ID details are required in the CD.

Format for Public Notice –

• It is for general information that I, ___________S/o __________________, residing at __________, have changed my name and shall hereafter be known as ______________. It is certified that I have complied with other legal requirements in this connection.
• In place of the signature- Type your Real Name(no sign), and no witness details are required on the CD.
• Only standard specimen format for Public Notice is required. No other documents such as application No, photographs, Or ID details are required in the CD

CD scanned in JPEG/PDF/Turkish format is not acceptable. The CD is to be saved in MS Word (Windows 2007) format only.
