    1. The matter of advertisement must be published in one of the daily local leading newspaper showing details of the case.
    2. An undertaking duly signed by the applicant showing therein correct & wrong name along with full details of father’s/husband’s name with residential address declaring that the facts stated therein are true & correct to the best of his/her knowledge and belief.
    In case of Indians living abroad, he/she will submit deed duly attested by the respective Indian Embassy/Indian High Commission, in original to this Department.

Printing matter as prescribed in the various attached specimen proforma duly complete in all respects by typing the same on a separate plain paper and signed by the individual in his / her name, with two witnesses in duplicate should be submitted along with Soft Copy (CD MS Word).     Updating Documents After Name Change in gazette

    After getting the copy of your Gazette Notification, you need to update your name on all documents.  Here are the few departments you need to visit:

    • Aadhaar Card: Go to the Aadhaar Enrollment Center with your gazette copy and other documents and get your name updated.
    • PAN Card: Apply for name change through NSDL website with required documents including Gazette Notification.
    • Passport: Submit application for re-issue of passport at Passport Seva Kendra with Gazette Notification.
    • Bank Accounts and Financial Instruments: Go to your bank with Gazette Notification to update your name on bank accounts, credit/debit cards and other financial instruments.
    • Property Documents: Update your name on property records by submitting Gazette Notification to local sub-registrar’s office.
    • Driving License: Apply for a name change at the Regional Transport Office (RTO) with the necessary documents.
    • Voter ID: Submit Form 8 along with the Gazette Notification to your local electoral office.

    Application Rejection and Reapplication

    If your Gazette name change application is rejected, you will receive a notification from Department of Publication, Civil Lines, Delhi-110054 stating the reasons for rejection. If your application is rejected, you can reapply after addressing the mentioned points in the rejection notification.
